Importing your Foil Drive to the USA - Form 5106

If you are ordering a Foil Drive directly from our website and you live in the USA, you will need to complete some paperwork when your parcel arrives in the USA.

To calculate expected importing fees, please read here.

Important things to remember:

- When you place the order on our website, make sure your email and phone number is correct.

- Check your spam or junk for emails from FedEx

- Complete the 5106 Form via the DocuSign link sent by FedEx

- SAVE YOUR COMPLETED FORM for your own reference 

Please note: You can also proactively send the form (i.e. before FedEx sends the DocuSign link) to FedEx once you have a tracking number from Foil Drive

1. Form is downloadable here: www.cbp.gov/sites/default/files/assets/documents/2019-Jun/cbp-form-5106.pdf
2. Fill out the form and create PDF of it and send to this email: [email protected]

a. In the subject line put ONLY the FedEx tracking number.

b. In the body of the email, do not write anything.
